WAIVER

Acknowledgment of Risk

I, the undersigned, acknowledge that participating in activities at Dinks and Drives, including but not limited to pickleball, golf simulation, fitness, or other recreational activities, involves inherent risks of injury, including the potential for serious bodily harm. I understand and voluntarily accept all such risks.

Release of Liability

In consideration for being allowed to use the facilities and participate in activities at Dinks and Drives, I hereby release and hold harmless Dinks and Drives, its owners, employees, contractors, agents, and affiliates from any and all claims, liabilities, damages, or causes of action, whether known or unknown, arising out of or relating to my participation.

Health & Medical Conditions

I affirm that I am physically fit and have no medical conditions that would prevent me from safely participating. I understand it is my responsibility to consult a physician prior to engaging in any activity.

Rules & Conduct

I agree to follow all posted rules and instructions from staff. I understand that failure to do so may result in suspension or termination of my membership or access.

Media Release

I grant Dinks and Drives the right to photograph, video record, or otherwise capture my likeness during activities for use in marketing and promotional materials, both online and in print.

Membership & Payment Terms

I acknowledge that this is a pre-membership authorization. My payment will be processed on or after May 10th, 2025, upon the official opening of Dinks and Drives. I understand that failure to complete payment may result in cancellation of my reserved membership.

By signing below, I acknowledge that I have read, understood, and voluntarily agreed to the above waiver and release of liability.
